How We Helped a Young Adult with Autism Transition to Supported Living Successfully

When the Martinez family first contacted GEM Support Services, their 22-year-old son Daniel was approaching a crossroads. He’d aged out of school services, still lived at home with parents who loved him deeply but recognized he wanted more independence, and had dreams of having his own apartment someday. The family was terrified—and hopeful. This is the story of how person-centered planning, gradual skill-building, and genuine partnership made Daniel’s supported living transition not just possible, but successful.


The Starting Point: Uncertainty and Untapped Potential

Daniel had aged out of special education services with solid self-care skills but limited experience with the practical demands of independent living. He could prepare simple meals but had never planned a week’s menu or managed grocery shopping. He understood money conceptually but had never paid a bill or budgeted for expenses. He craved social connection but relied entirely on family to facilitate it.

His parents, Maria and Carlos, worried constantly. Could Daniel really live on his own? Would he be safe? Would he be lonely? Would they be abandoning him by encouraging independence? These fears are universal among IDD families approaching transition—and completely understandable.


The GEM Approach: Gradual, Person-Centered Skill Building

Rather than rushing toward an apartment, we started where Daniel was. Our Supported Living Coach began visiting the family home, building relationship with Daniel and assessing his current abilities against the skills independent living would require.

Together with Daniel—not just his parents—we identified goals that mattered to him. He wanted to cook meals he chose himself. He wanted to take the bus to his favorite store without his mom accompanying him. He wanted to have friends over to “his place.” These weren’t our goals imposed on Daniel; they were his dreams articulated in his own words.

We broke each goal into teachable steps. Meal planning started with choosing between two options, then expanded to planning three dinners, then a full week. Bus training began with accompanied rides, progressed to Daniel leading while a coach observed, and eventually became independent travel with check-in texts.


The Transition: Moving Day and Beyond

After eighteen months of preparation, Daniel moved into a supported living apartment. It wasn’t a sudden leap—we’d practiced overnight stays, established relationships with neighbors, and ensured Daniel knew exactly what to expect. His GEM coach continued providing in-home support, now in Daniel’s own space rather than his parents’ home.

The first months required adjustment. Daniel experienced homesickness. He made mistakes—an overflowing washing machine, a burnt dinner that triggered the smoke alarm, a missed medication dose. But with consistent coaching support, each mistake became a learning opportunity rather than a crisis.


Where Daniel Is Now: Thriving on His Terms

Two years post-transition, Daniel’s life looks remarkably different. He hosts a monthly game night with friends he’s made through community programs. He takes pride in his apartment, decorating it with his interests. He’s exploring supported employment options and recently started a part-time position.

His parents describe a transformation in their own lives too. Their relationship with Daniel has deepened now that it’s no longer dominated by daily care logistics.
